Your First Visit

We kindly ask that you arrive about 10 minutes early on your first appointment or fill out the form online below.

Initial Examination

Your dentist will begin by reviewing your medical and dental history and completing a comprehensive dental exam. Your exam will include a thorough examination of your gums, teeth, bite and jaw, an oral cancer screening and any x-rays that may be required.

Treatment Plan

Based on your history and examination results, your dentist will develop a long-term dental treatment plan for you. Your dentist will also take the time to answer any questions you have about your oral health and treatment options.

• How can I book an appointment?

  To book an appointment at Cornerstone Dental Group - Dr Rex Hawthorne and Associates, please contact our office during our regular business hours.

• Does your clinic have on-site parking?

  Yes, we have free on-site parking available for all our patients. We are also easily accessible by public transit. 

• Is your office wheelchair accessible?

  Yes, our office is wheelchair accessible and we are happy to welcome patient's requiring wheelchairs. 

• How do I cancel or reschedule an appointment?

  If you find that you need to cancel or reschedule an appointment, please call us. Please let us know about any change of plans as soon as you can, so we can offer the space in our schedule to another patient.

• Do you offer appointment reminders?

  In the days leading up to your appointment, you will receive automated phone, text or email appointment reminders from us. If you need to cancel or reschedule an appointment, please call us as soon as possible to let us know.

• Does your office provide free wifi for patients?

  Yes, we are happy to provide our patients with free wifi. To log onto our service, ask our reception team for details.

• Do your treatment rooms have TVs?

  To help make your visit to our office a little more fun, our treatment rooms are also equipped with Smart TVs with both, cable and Netflix! 

Insurance & Financing

At Cornerstone Dental Group - Dr Rex Hawthorne and Associates, we'll help you navigate our options for payment, direct billing to insurance companies and financing so you can focus on getting the dental care you need.

• Direct Billing to Insurance

  We accept payment directly from most insurance companies (assignment of benefits). For the ones we don't, we are happy to complete the required forms and send in your insurance claim for you. Your dental insurance company will then pay you directly.

  Our administrative team can assist you in understanding your insurance plan better and send estimates to your insurance company prior to starting any major treatments. 

  Please bring your insurance plan information to your appointment.

• Financial Policies and Arrangements

  We believe you should be made aware in advance of the cost of treatment; no one likes surprises.

  We can help make dentistry affordable by suggesting options for major treatment that fit your budget .

• Payment Methods

  Please note that any co-pays or amounts not covered by insurance are your responsibility, and must be paid at the time of your appointment.

  We accept payment by cash, debit or most major credit cards.
